How to Mix Tirzepatide
Reconstitution Guide: BAC Water Ratios & Dosing Chart
Always consult a healthcare professional before starting any injection protocol.
Why Reconstitution Matters
Tirzepatide is supplied as a freeze-dried powder and must be reconstituted with bacteriostatic water (BAC water) before injecting. Getting the ratio wrong changes your solution's concentration, which throws off every unit dose you draw afterwards.
📊 Reconstitution Ratio
| Vial Size | BAC Water Added | Final Concentration |
|---|---|---|
| 10mg vial | 1.0ml | 10mg/ml (100 units = 10mg) |
🧪 Step-by-Step Mixing Instructions
Wash your hands
Wash thoroughly with soap and water for at least 20 seconds and dry with a clean towel.
Swab both vial tops
Clean the rubber stopper on your Tirzepatide vial and your BAC water vial with a fresh alcohol swab each. Let air-dry for 10 seconds.
Draw 1.0ml of BAC water
Using a syringe, draw exactly 1.0ml of bacteriostatic water from its vial.
Inject slowly down the vial wall
Angle the needle so the water runs down the inside of the glass onto the powder — do not squirt it directly onto the powder.
Check the solution is clear
It should be clear and colourless with no particles. Discard if cloudy or discoloured.
Label and refrigerate
Write the mixing date on the vial. Store at 2–8°C and use within 28 days.
📅 Dose to Unit Conversion (10mg vial, 1.0ml BAC)
| Dose | Units (U-100) | Volume |
|---|---|---|
| 2.5mg | 25 Units | 0.25ml |
| 5.0mg | 50 Units | 0.50ml |
| 7.5mg | 75 Units | 0.75ml |
| 10.0mg | 100 Units | 1.00ml |
| 12.5mg | 125 Units | 1.25ml |
| 15.0mg | 150 Units | 1.50ml |
Tirzepatide Mixing FAQs
How much BAC water do I add to a Tirzepatide vial?
Add 1.0ml of bacteriostatic water to a 10mg vial, giving a 10mg/ml concentration. On a U-100 insulin syringe, 100 units equals 1.0ml equals 10mg.
Can I use sterile water instead of BAC water?
No. Sterile water contains no preservative and the mixed solution must be used almost immediately. Bacteriostatic water allows safe storage in the fridge for up to 28 days.
What if my dose is higher than 100 units?
A standard U-100 insulin syringe caps out at 100 units. For doses of 12.5mg or 15mg, use a 3ml syringe with clear ml markings, or draw your dose across two separate syringes.
How long does mixed Tirzepatide last?
Once reconstituted, store in the fridge at 2–8°C and use within 28 days. Label the vial with the mixing date.
